Case Management Services

Northern Nevada Medical Center's case management department is available to help patients and their families cope with the problems and stress that often accompany illness or hospitalization. Nurses and professional social workers provide support and guidance in dealing with an illness or treatment, and can help patients make decisions about their discharge plans. They are experienced in working with community agencies that can provide patients and families with the services they need, including information on extended care facilities that may be needed following discharge from the hospital.
